Rules Related to Madhya Pradesh State Disease Assistance Fund

Financial assistance ranging from ₹25,000 to ₹2,00,000 for major surgeries or medical interventions.

NOTE:

The SIF assistance amount cheque is provided directly to the Hospital/Institution where the approved case (patient) is referred.

The grant will be permissible for the recognized hospitals within the state & outside the state.

For cancer cases, the grant will be given to the JLN Cancer Hospital, Bhopal.

The applicant must be a domicile of Madhya Pradesh.

The applicant must belong to a Below Poverty Line (BPL) family.

The applicant must be suffering from a disease listed among the approved illnesses under the scheme.

The hospital must be a recognized institution within or outside Madhya Pradesh.

Only one eligible family member can receive assistance once.

Offline

Step 1: The interested applicant downloads the application form

from the official website.

Step 2: In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attest, if required).

Step 3: Submit the duly filled and signed application form along with the documents to the respective District Collector, addressed to the Director, Public Health and Family Welfare.

Residence Certificate (Issued by Collector/Sub-Divisional Magistrate)

Below Poverty Line (BPL) Certificate

Medical Diagnosis Certificate (Issued by the Civil Surgeon)

Treatment Estimate from a Recognized Hospital

Referral Documents (If the case is referred to specific medical institutions)

Any Additional Medical Reports required by SIAF during evaluation
